The clinical experience has been successful so far. I experienced success and some challenges this past week. Some of the successes I experienced included learning about new approaches to disease management, collaborating with other healthcare providers in the provision of patient care services, and involving patients and their families in decision-making. I educated patients and their families on health promotion and disease prevention strategies to improve health outcomes. I also incorporated care strategies such as shared-decision making and patient involvement in the disease management process. I also collaborated with other healthcare providers in assessing, planning, implementing, monitoring, and evaluating care given to patients. I believe that my teamwork knowledge and skills have improved significantly since I started the clinical experience.

I am also learning how to translate evidence into practice. I am relying on clinical practice guidelines and best practice recommendations to make treatment decisions for my patients. I experienced a challenge in the past week. I encountered a family that was against immunization. The family did not want their child to be vaccinated since it was against their religious beliefs and practices. The family declined immunization even after I educated them on its benefits and low health risks. I respected the family’s decision for autonomous care.
I reviewed an 8-year-old male client whose parents brought him to the facility for assessment and treatment. The presenting complaints included the presence of red, circular, itchy rashes on his right face and neck for the past three days. The patient reported that the red ring had slightly raised edges and itchiness that worsened with skin exposure to sweat. His parents denied the use of any medications for symptom relief. Physical examination revealed the presence of red and circular rings on the right face and neck of the client. The rings appeared scaly with scattering, red pumps. There was no evidence of scratching or bleeding from the rings. The findings led to a diagnosis of tinea corporis to the right face and neck.
Some differential diagnoses were considered for the above client. They included cutaneous candidiasis, contact dermatitis, and psoriasis. Cutaneous candidiasis is a fungal infection that affects warm, creased, or moist skin areas such as the groin. Patients usually experience symptoms such as skin rash, redness, and itchiness. However, there is no formation of skin rings, as seen in tinea corporis. Contact dermatitis is a skin condition that develops when a patient is exposed to an allergen. Contact dermatitis is associated with symptoms such as a skin rash, itchiness, and formation of blisters or bumps. Unlike in tinea corporis, patients with contact dermatitis do not have skin rings. Psoriasis is an autoimmune disease of the skin characterized by abnormal multiplication of the skin cells (Chen et al., 2021; Leung et al., 2020). Patients have inflamed scaly skin. However, they do not have itchy skin rings, as seen in tinea corporis.
The patient was prescribed clotrimazole 1% ointment to be applied topically on the affected skin areas twice daily. The patient and the family were educated on the importance of wearing loose-fitting or light clothing, keeping the skin dry, and treating compliance. They were also educated that treatment results would not be immediate (Yee & Al Aboud, 2024). This week, I learned the importance of active patient and family involvement in the care process, health promotion and disease prevention interventions, and interprofessional collaboration for optimum care outcomes.
