The clinical experience has provided me with considerable learning opportunities. I have experienced success in my clinical experience. I have assessed several pediatric clients during my clinical experience. I will share a recent experience where I assessed and diagnosed a pediatric client with acute otitis media. The patient was a 4-year-old male whose mother brought him to the facility with complaints of ear pain, irritability, lack of sleep, decreased appetite, and mild fevers. The additional complaints included pulling the affected ear and headaches. The mother denied ear discharge. These symptoms started two days before the clinical visit and were mildly responsive to over-the-counter Tylenol given at home.
A physical assessment was performed on the patient. He appeared well dressed for the clinical visit, alert, and oriented, with a mild fever. An otoscopic examination was performed on the affected ear. The findings included an erythematous tympanic membrane, some fluid in the middle ear space, and an edematous external ear canal. Laboratory and imaging studies were not done since the physical assessment findings confirmed acute otitis media. The patient’s treatment included the prescription of antibiotics and analgesics for pain management (Danishyar & Ashurst, 2024). The selected treatments included oral amoxicillin 5 ml twice daily for ten days and oral Tylenol 5 ml thrice daily for five days. A follow-up visit was scheduled after five days to determine the patient’s response to the treatment.
Some differential diagnoses were considered for this patient. They included pediatric mastoiditis, otitis externa, and pediatric allergic rhinitis. The absence of symptoms such as rhinorrhea, sneezing, and watery eyes rules out pediatric allergic rhinitis. The absence of ear drainage ruled out otitis externa. The absence of a recent ear infection in the child’s history ruled out pediatric mastoiditis. Health promotion messages were shared with the mother. They included educating her on the importance of treatment adherence, avoiding inserting foreign materials into the ears, the side effects of the treatments, and ensuring that her child’s immunizations were up-to-date (Hayashi et al., 2020). I learned from this experience the importance of comprehensive patient assessment. A comprehensive patient assessment helps develop accurate diagnoses and treatment plans for patients. I also learned the importance of patient or family involvement in the care process. Patient and family involvement improves treatment adherence and sustainable change in health behaviors and lifestyles.
