Nurses play crucial roles in the promotion of optimum health outcomes for the communities they serve. They collaborate with community members to implement strategies that align with community’s values, practices, expectations, and needs. In doing so, nurses utilize different models for program development. I would use the screening and health educator model when implementing community health programs. The screening and health educator model recognizes the vital role that nurses and community health workers play in delivering screenings and health education to community members (Ruralhealthinfo.org, n.d.). Their role aims at promoting the health and wellbeing of the community members. The screenings and health education may focus on community issues such as chronic disease prevention, nutrition, physical activity, stress management, and home health and safety. I would adopt this model since it focuses on raising population’s awareness about their needs, how to meet them, and their abilities. Its effective use can also lead to sustained change and outcomes since it targets behavioral change among community members (Harrison et al., 2019). The model also empowers community members to take responsibility of meeting their diverse health-related needs.
Program sustainability and success are important when addressing community needs. One of the ways to achieve these outcomes is by ensuring the program addresses the actual community needs. This ensures relevance of the program to the community. The other approach is by aligning the program strategies with community expectations and resources. Alignment enables the use of the available resources to achieve optimum outcomes. The last strategy is active involvement of community members. Active involvement is an essential component that promotes ownership of the program by the community members. It also promotes sustainability since the adopted strategies align with the community expectations and needs (Alison, 2019).

The Affordable Care Act is one of the health reforms adopted in America to increase access to healthcare for the population. The policy addresses barriers to healthcare due to lack of medical insurance coverage and costs. The decision to have the option of insurance in both the private and public sectors will have significant implications on population health, healthcare institutions, and public health spending by the government. First, having the option for insurance coverage in both the private and public sectors benefits the population by increasing their healthcare access. The vulnerable populations will be assured of an expanded access to high-quality care whenever they need. There will also be an increased spending on healthcare by the population (Davidoff et al., 2018; Hu et al., 2018). The expanded coverage in both sectors enhances the utilization of health promotion services, which increases healthcare spending and health of the population.
The government’s spending on these insurances is also likely to go up due to the increased service utilization by the population. However, indirect benefits such as an increase in productivity in the country will help in offsetting the potential cost implications of the dual insurance in both sectors. The other potential implication of the dual insurance coverage is that health organizations will face the challenge of having to meet the changing care needs of the population. Accordingly, the expanded access to healthcare for the population will enable the vulnerable to access the care that would otherwise be a challenge for them. As a result, it will shape the discussion about population-based nursing. Policymakers will recognize the need for the active involvement of nurses in the implementation of population-based health promotion initiatives. There will be a shift of focus in nursing and healthcare from disease management to prevention (Hendryx & Luo, 2018). The potentials for increased investment on health promotion initiatives will increase because of the need to minimize and prevent avoidable diseases in the population.
