Overview
In this Performance Task Assessment, you will collect data associated with your practice problem in LC4002 and look for trends. You will either confirm or revise your practice problem, then apply a quality improvement process to it.Looking Ahead
Over the course of Competencies LC4002A – LC4005A, you will be going through the steps of your Quality Improvement Project which will be submitted in LC4005A. During each Competency, you will complete one or more parts of the final project, and in LC4005A, you will use these parts to create the final project.This practice experience project has six parts:Analyze available evidence and identify a quality improvement practice problem;
Conduct a literature search finding data and evidence-based solutions/interventions to support the importance of the quality improvement practice problem;
Identify the quality improvement tools (Fishbone diagram, Pareto Chart, Process Flow Chart, etc.) that will display the data supporting the need for the Quality Improvement Project;Order LC4003A Quality Improvement Processes
Apply the PDSA process to the development of the Quality Improvement Project;
Develop an evidence-based plan to address the quality improvement practice problem; and
Present the final Quality Improvement Project as a comprehensive paper and quality improvement storyboard.
The practice experience is an active learning process that provides you with the opportunity to apply your nursing knowledge by addressing a quality improvement practice problem. The practice experience is comprised of selected onsite experiences and completion of a Quality Improvement Project with guidance from your subject matter expert. Instructions
To complete this Assessment, do the following:

Be sure to adhere to the indicated assignment length.

Access the following: LC4003A Assessment Template (Word document). You will use the Assessment Template to make your submission.
Before submitting your Assessment, carefully review the rubric. This is the same rubric the SME will use to evaluate your submission and it provides detailed criteria describing how to achieve or master the Competency. Many students find that understanding the requirements of the Assessment and the rubric criteria help them direct their focus and use their time most productively.

All submissions must follow the conventions of scholarly writing. Properly formatted APA citations and references must be provided where appropriate. Submissions that do not meet these expectations will be returned without scoring.

This Assessment requires submission of one (1) document, a completed Assessment Template, approximately 2- to 3-pages. Save the file as: LC4003A_firstinitial_lastname (for example, LC4003A_J_Smith)

LC4003A Quality Improvement Processes

In part three of the template the assessment instructions say apply the PDSA model- what this means is that you write your entire plan for improvement using PDSA as a framework. Use the information I have provided here to develop your quality improvement plan. The PDSA plan needs to set up so that whoever is reading your plan could implement that quality improvement plan, so lots of detail, (and every intervention used in your plan should be cited demonstrating evidence of success for improving your problem in the literature. I should be able to find in your lit/evidence review from 4002.

You will need to describe in detail each phase of your PDSA model Be specific with each phase of the model- use a heading for each phase of the model- As you are only writing the plan and not implementing you will not be able to be as thorough in the Study and Act phases of the model, but you will still need to address using a future tense. Once this section is complete you will cut and paste into your capstone paper in 4005 🙂

PLAN: Plan for change/test/intervention (provide a citation from the literature for each intervention, from 4002) Include the following What are you trying to improve, by how much (goal) and by when (time frame? ) Who (target population) What will you do (change/test): When? (when will you begin? How long will your test of the intervention last?) Where (location):

How? describe who will carry out the intervention you are using – don’t forget you will likely need to educate staff too- Provide a plan for this too. Plan for data collection Who (will collect the data): What will you measure? When ?(what time period): How (How will you collect the data)

DO: You can simply state here that the plan will be implemented. Also tell me about data collection who and how will data be collected data as the testof your intervention progresses? STUDY: This is where you will tell me that you summarize and analyze data you collected in the Do phase ACT: This is where you will tell me what you would do if you met your goals What happens next? What will you do if you meet your goal? What will you if you don’t meet the goals? Remember PDSA is cyclical, never one and done- In section three you are going to describe your type of measure outcome, process or structural and then describe how you will know that your plan was successful. Be specific on your measure of success- is a process, structure or outcome measure?
